Job Description

Annual Salary: $52,000
Position Summary: This professional position serves as the centralized point of contact for all new placement requests for children and youth requiring Licensed Out of Home Care Placements.  This position will serve multiple functions which will include participation in STC & FTC staffing’s, Multi-Disciplinary Staffing’s, Placement Stabilization Staffings and Shelter Support Calls. In addition, this position will act as a support for children and youth requiring a staffing through the “Every Student Succeeds Act” (ESSA) to ensure educational stability for children in Out of Home Care .  

Technical/Functional Expectations:
  1. Centralized Point of Contact-Essential Function: This position fields calls from Protective Investigators for new cases and accepts request for licensed placement,  
  2. Placement Identification in Licensed Care-Essential Function: Networks with our Brevard Family Partnership’s licensed foster homes as well as any Child Placing Agencies to identify and secure appropriate placement setting for children in licensed out of home care. Which also includes working in collaboration with MFC to ensure children requiring MFC placement have all the required medical records/CMAT paperwork needed to determine placement needs.
  3. Maintenance of Daily Movements Essential Function: Collect data as applicable to children in licensed out of home care to ensure accuracy.
  4. Maintenance of Overcapacity Waivers Function: Complete and Track all overcapacity waivers and ensure paperwork is accurate and updated with in the required timeframes.
  5. Educational Review: Facilitate the ESSA process for all youth who enter Out of Home Care through providing the school of origin, the CPI/DCM and the appropriate Brevard Public Schools Point of Contact with the required documents that are to be completed. The ESSA process includes but is not limited to, coordination of the meeting within 2 business days of notification, transportation support, communication with all members of the child’s team to help identify and assist with any barriers, completion and submission of the determination paperwork in all required forums within 5 business days of receiving the ESSA paperwork.
  6. ResWrap: Acts as a companion for youth in group care to advocate and lead step down efforts by completing the All About Me and participating in all ongoing staffing’s. 
  7. Point of Contact for all Crossover Youth – Works as a liaison with the Department of Juvenile Justice to ensure children that are dually served and at risk of removal are identified and receive appropriate referrals and services.
  8. On Call Rotation: Participate in the first tier on call rotation in order to provide timely service delivery.
  9. Customer Service: Work with clients and families to trouble shoot and assist with problem solving. To ensure client satisfaction.
  10. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting and training others, and performing duties of higher rated positions from time to time for developmental purposes.

  12. Strong communication skills: This is essential in order to develop written summaries and recommendations for distribution within the system of care. Intermediate to advance level skills in MS Office to include Word, Excel and PowerPoint and familiarity and comfort using databases to populate and evaluate data, make case notes and pull data.

Business Experience:

  1. Valid Driver's License-Essential: This position requires a valid driver’s license.
  2. 5 years’ experience in child welfare or human svc experience: This position requires at least 5 years of experience in the intake of children in the child welfare system or human services field.
  3. Computer skills required: This position will be required to work in Microsoft Word, Excel and Access.
  4. Excellent Analytical and Problem-Solving Skills: This function requires the ability to independently assess, evaluate, and make recommendations regarding placement and services for children in licensed out of home care.
  5. Child Protection Certification: Certification in Child Protection or eligible for certification preferred.

Educational and Experience Requirements:
1.       BA/BS - bachelor’s degree or equivalent required.
2.       MA/MS - master’s degree or equivalent preferred.
3.       2 to 5 years related experience in child welfare required.
4.       5 to 8 years related experience in child welfare preferred.
